Medical Transport Term

Signed trip record

Definition

The per-trip file behind one billed ride, down to the rider's signature on a non-emergency leg.

Why It Matters

Recurring authorization makes standing dialysis trips the backbone of the book. Every repeat on that authorization still has to produce its own record. Ohio requires the vehicle used, the attendant's name, the rider's Medicaid number, and for non-emergency trips the signature of each individual transported, and it says no payment is to be made where the provider fails to obtain the documentation before submitting the claim. Two other sentences do the damage together: records are kept six years, audits may use statistical sampling, so one missing signature on a sampled trip is extrapolated across the population. Failure to supply records within thirty days withholds payments outright. Settle in the agreement who answers a lookback on the seller's years.
