The five-year re-enrollment every Medicaid provider must clear, whoever owns it.

Why It Matters

It decides whether a buyer inherits four years of runway or four months. A target mid-cycle is a target whose next full screening lands on the new owner, and a change of ownership is itself treated as a fresh application in many states, with its own fee per service location and in some a surety bond. Read the enrollment file for the date, not the seller's memory of it, and put the fee and the bond in the sources and uses.
