# DIR fees

A term used when buying pharmacy.

A benefit manager's price concession, clawed back after the fill until 2024 and now taken up front.

Read the date before the number, because the mechanism moved. Until 2024 these were clawed back weeks after a fill, so a profitable prescription could become a loss and gross reimbursement was not a figure anybody could underwrite. From January 2024 the price concession has to show at the point of sale, which makes the remittance a figure you can underwrite and the trailing years incomparable. Normalize the seller's earnings across that line, and treat any three-year trend that ignores it as a chart of two different things.
